- We seek dynamic nurses or advanced paramedics with amazing interpersonal skills to support the leads in the county wide Urgent Community Response team. We are looking to recruit new team members who can proactively and effectively deliver a range of treatments, including IV therapy and all aspects of clinical care in the patientβs own home environment.
- β’ bringing care closer to home and we also provide dedicated nurses into the front doors of both our acute hospitals in the county.
The Urgent Community Response team operates as a Virtual Ward across the county, managing patients with sub\-acute conditions to aid their recovery from illness or injury within their own homes by preventing unnecessary hospital admissions and reducing length of hospital stay.
In order to respond to the healthcare needs of the local population, the teams operate 7 days a week ensuring the delivery of the service can respond to patientsβ needs as and when required. The shifts for the service are between 07\.00am to 01\.00am including weekend and bank holiday working as part of the rota. The successful candidates will be expected to work within these shift patterns.
We are looking for experienced RNβs or advanced paramedics with a passion for high quality clinical care and also experience in either acute medicine, Emergency Care or Community Nursing. The post holder will provide acute assessment \& interventions in patientsβ own homes and will ensure that patients are referred onwards as appropriate to community services once their care episode is complete. To be considered You must have: \-
Excellent organisational ability
Senior nurses/paramedics band 6/7 with excellent leadership skills
Superb level of communication skills
Able to flex to a changing environment for service improvement
Good working knowledge of digital communication products
Real interest in improving \& delivering healthcare in the community.
Clinical triage skills
High level assessment, diagnostic and clinical skills
Prescribing or willing to work towards
Be capable of working under pressure across a variety of treatments
