Responsibilities
- Links patients/families to essential support services including housing, nutrition assistance, utilities, and transportation
- Conducts follow-up calls to patients and families to confirm linkage with community resources
- Provides appropriate documentation of patient and family interactions in EHR
- Develops expertise in making referrals to housing resources, food pantries, and treatment facilities
- Assists clinical staff and partners in decision-making for improving patient linkages
- Collects and reports data on program activities
- Reviews potential resources that are culturally sensitive to the community
Requirements
- Community Health Worker certification by the Ohio Board of Nursing or willingness to complete it within 6 months of hire
- 1-3 years of related professional experience with social service agency, community-based program, or health-related institution
- Experience working and/or volunteering with children and/or families
- Strong knowledge of health disparities and maternal/infant health issues
- Basic computer skills
- Experience working with diverse community groups

PrimaryOne Health is currently seeking a Women's Community Health Worker for our East Main St. health center.

Essential Job Responsibilities:

Addresses identified patient and family SDH needs and improves patient/familyβs access to community resources.
------------------------------------------------------------------------------------------------------------------
In collaboration with clinical staff, links patients/families to essential support services including housing, nutrition assistance, utilities, transportation, and other needs as identified by the patient/family and clinic team.
----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------
Assists in conducting follow\-up calls to patients and families to confirm linkage with identified community resources and/or services, assesses potential barriers, and aids in navigating resources and services. May accompany patients and families to social service appointments.

Follow\-up with clients by letter, phone call/text, or home visit on missed appointments or referral.

Provides appropriate documentation of patient and family interactions in EHR in a timely manner.

Develops an expertise in making referrals to housing resources, food pantries, utility companies, government agencies, treatment facilities and other internal and external resources, including understanding programsβ eligibility criteria, intake process, etc.

Assists clinical staff, supervisor, and organizational partners in the decision\-making process for improving linkage of patients/families with community resources.

Reviews and recommends potential resources that are culturally sensitive to the targeted community.

Remains informed of trends, developments, as well as state\-of\-the\-art prevention efforts and strategies, and general information regarding the targeted community.

Successful candidates will possess a Community Health Worker certification by the Ohio Board of Nursing or willingness to complete Community Health Worker certification within 6 months of hire. 1\-3 years of related professional experience with social service agency, community\-based program, or health\-related institution. Experience working and/or volunteering with children and/or families required. Work is generally independent and collaborative in nature. Strong knowledge of health disparities and maternal/infant health issues. Knowledge of some OB/GYN medical terminology. Basic computer skills. Experience working with diverse community groups.
