    Manitoulin\-Sudbury District Services Board (DSB) is looking for a self\-motivated \& energetic individual to join our industry\-leading organization which works towards the delivery of high\-quality services and programs to the residents of Manitoulin\-Sudbury districts. Created by the provincial government, the DSB is responsible for overseeing and managing a range of services divested to the municipal order of government, including Ontario Works, Community Housing, Paramedic Services (Land Ambulance), Non\-Urgent Patient Transportation, and Early Learning and Child Care Services.

    Our DSB is looking for your help to ensure the efficient delivery of services to the residents of Manitoulin\-Sudbury districts.     Our Team

    The Manitoulin\-Sudbury District Services Board is comprised of 14 board members and has been in existence for 25 years. With three main departments, Integrated Human Services, Paramedic Services, and Administration, and over 200 employees, we service a large geographical area totaling 45,000 square km. Our organization is responsible for the development and execution of annual budgets exceeding $50 million. We are proud to serve our communities, and we remain committed to our mission of delivering effective, fair, and safe services to our residents.

    The Opportunity

    We are currently looking for a driven individual to join our team as a Temporary Services Navigator within our Integrated Human Services Department.

    Term of employment: to end of March 2026

    Location: Mindemoya – with travel


    In this position you will:
    Assess individual needs, provide information to support individual improvement plans, evaluate the plan’s ability to improve outcomes.
    Ensure personal safety in partnership with the Community Paramedic to determine if it is safe to enter the individual’s living space, identify if the team will enter the home.
    Collect and provide statistical data; to support operational needs, evaluation by consumers of services provided, and information on all resources by community.
    Investigate alternative methods or partnerships for service opportunities and pathway development to optimize efficiency and improved outcomes. (patient, client, person)
    Support individuals in collaboration with Community Paramedic to improve overall health and address social needs.
    Refer individuals to services and support that promote the achievement of goals.
    Work and utilize all Manitoulin\-Sudbury DSB programs, using an integrated service delivery approach.
    Build and maintain strong connections with internal and external stakeholders.
    Promote excellence in case management. Document the services and supports provided using an outcome plan.
    You must wear personal protective equipment as required and issued by the employer.
    Maintain current knowledge and awareness of relevant legislation, policies, professional human service techniques and practices, community services, support, and opportunities that benefit individuals.
    Identify and communicate gaps in service delivery to the Commander of Community Paramedicine and recommend improvement strategies.
    Ensure that data collection, verification reporting, privacy and audit requirements and any statistical reports are completed as required. Participate in approved professional and organizational development programs.
    Perform other duties as assigned.
    Qualifications and Specific Skills


    Education:
    Post\-secondary education from an accredited Canadian University or College in Human Services or similar, or a combination of education and/or experience is required.
    Must possess Standard First Aid and CPR.
    Experience

    Minimum of 3 years of experience in Human Services with an emphasis on program delivery.
    A valid Ontario Class G Driver’s License with insured vehicle for employment related use is required.

    Bilingualism (English and French) is considered an asset.


    Computer Skills:
    High level of ability to use computers and other information technology specific to case management and administration.
    Extensive knowledge of Microsoft Office, including Outlook, Word and Excel.

    Salary: $35\.78/hr (35 hours per week)
